Background and purpose: The use of electronic prescriptions is essential for improving healthcare quality and maintaining patient well-being. Despite numerous advantages, such as increasing speed, accuracy, and enhancing service quality, electronic prescription writing has faced non-acceptance and resistance from healthcare providers and users. By identifying the influential elements that contribute to the non-acceptance of electronic prescriptions, corrective actions can be taken. Therefore, the present study aimed to investigate the challenges of electronic prescription writing at Hazrat Nabi Akram (PBUH) Hospital in 2025.
Methods: This qualitative study was conducted in 2025 at Nabi Akram (PBUH) Hospital in Assaluyeh. The study population included physicians, pharmacy staff, and pharmacists. Purposive sampling was employed until data saturation was reached. Semi-structured interviews were conducted with 12 physicians, 3 pharmacy employees, and 2 pharmacists. Qualitative content analysis was used to analyze the data.
Results: The findings identified three main categories of challenges affecting electronic prescribing: organizational challenges, systemic challenges, and patient-related challenges. Organizational challenges included issues related to policymaking (8 cases) and monitoring (3 cases), as well as physician awareness challenges (3 cases). Systemic challenges comprised infrastructure problems (5 cases) and issues concerning the use of the insurance system (6 cases). Patient-related challenges included patient awareness (3 cases).
Conclusion: The implementation of electronic prescribing in Iran has not progressed as expected compared to its early years and continues to face numerous challenges, including organizational, systemic, and patient-related barriers. It is suggested that policymakers and health system managers focus on improving infrastructure, integrating insurance systems, implementing electronic signatures, complying with electronic prescription standards, and providing practical training to physicians.
